    On the print server (if stand-alone, the workstation):

    1. Open the Printers interface.

    2. Click on File and "Server Properties"

    3. On the "Forms" tab, put a check in the box "Create a new form"

    4. Create your new form size and give it a name.

    5. Click "OK"

    6. On your printer, choose your new form name.
