USB 2.0 and ghost devices

I attached a mobile phone to a USB 2.0 5-Port PCI card. The phone tried to
install itself as a modem on the COM3 port. I didn't have the drivers for
the phone so it was
never fully installed. Even after unplugging the phone, my computer insists
there is still a device there. An unknown PCI device shows up and the
computer wants to install the drivers for. I cannot get rid of the device.

I have the following setup:
Windows 2000 SP4
Intel Pentium 4 1.8Ghz
Intek USB 2.0 5-Port PCI card

I have tried the following:
1) Uninstall the PCI device from the Device Manager Menu. The computer
plug-and-play discovers new hardware when it is rebooted and the unknown PCI
device reappears.
2) Uninstall Hardware wizard. Same result as no. 1 above.
3) Uninstall all drivers for the PCI device and the USB 2.0 5-Port Adapter
and then physically remove the USB 2.0 5-Port Adapter from my PC. When the
PC is rebooted, the computer reinstalls all of the drivers for the USB 2.0
card and finds
the mobile phone device as a PCI device and tries to reinstall it. This is
amazing to me because the phone and the USB card isn't even connected to the
computer!
4) I have found the entries for the phone in the registry however it will
not allow me to delete them because the system has the keys locked, of course.
5) I have started the computer in Safe Mode with similar results.

I don't know what else to try to get rid of the phone and unhook it from
COM3. I just want the damned thing gone. Please let me know how I can fix
this.
