It's not completely clear what you are saying is
happening.

First guess would be a name resolution or authentication
problem where the client doesn't get the profile reliably
FROM THE SERVER, and uses a local cache, then
returns to the server.

One profile overwrites the one with the certificate.

These sorts of problems might account for it working
in the lab and failing on the main network.
