I had the same problem but discovered that the Civ 4 installation overwrites Direct X 9.0c used by Vista. The DirectX 9.0c files with the game are old and actually cause the problem. I downloaded the distributable version of DirectX 9.0c from here:-

Once installed my Civ 4 worked flawlessly.

I am running Windows Vista and cannot get Civilization to load. I have
spent 4 hours installing and uninstalling with tech support (Downloaded
all recent updates) and still cant get it to work. Has anyone else had
this issue happen to them?

Vista doesnt give me an option to investigate the problem further.
