Adding a Control to a Dialog

My MSDN Library Visual Studio 6.0 gives the following advice on
CColorDialog:

To customize the dialog box, derive a class from CColorDialog, provide
a custom dialog template, and add a message map to process the
notification messages from the extended controls. Any unprocessed
messages should be passed to the base class.

How do I "provide a custom dialog template" when I have no details of
the standard dialog box, such as control design and object IDs ?

This is something I have been puzzled by for 10 years, but always
ducked the issue.
