The first, headers-only page shows report, page and one group header.
If I hide that group header the report prints out correctly so I'm not
sure if it's that header or the Details sections that are causing the
problem.

Any ideas?

Have you checked the 'keep group together' option in the group window,
not the section one, do a change group and check in there.
