Thanks. I have been through most posting about this problem, but no luck.
It appears everything is working just fine. I am not sure if my thinking is
correct, but the eventcache table gets populated and then the item is
removed. Can I assume the timer service is working correct? Searching also
works inside the site.
With that said, I am trying to locate further debugging ideas to help narrow
it down. I have been through the basics - proper permissions, resetting
account id/password for the heck of it, etc...

Well you could look at the various "why are Alerts not working" items I
have in the WSS FAQ (the 2.0 List) here

(just search on "alerts")
The reason I mentioned the port 25 thing is because that was typically a
reason for something *stopping* from working that was working before.
The other immediate possibility is that someone or something has turned
off the SharePoint Timer Service.
If that's still running - have a run through what other things that WSS
FAQ List offers.

If it was WSS 2.0 I would suspect the addition of a virus checker that
is blocking port 25.

1. Have a windows explorer openened to the 12 hive\LOGS sorted by
modification date.
2. Do something that should generate the alert.
3. Open up the most recently modified log file in notepad.
4. Copy/paste into excel.
5. Jump to the bottom and hope you find a useful error that you can
use to proceed with more specific research.
