Forms Based Auth Problem -WSS3.0

Hello All
I have managed to enable forms based auth on a host named site but
cannot get access to it anymore. When I try and login it states access
is denied for me and then gives me the option to request access. When
I run central admin and look at the site administrator it now states
no exact match found and if I attempt to use the people picker I
cannot select anyone as they dont appear.
STOP PRESS! - I have now managed to add a new site administrator from
central admin by entering the name in the following format:
ADProviderName:Account
However when I try to log into this site using the same format as
above I get the following:
The server could not sign you in. Make sure your user name and
password are correct, and then try again
Can someone help me solve this problem?
Also is it true that you only get the option to create users as a site
admin if you have enabled forms based auth (This is the only reason I
did it cos my ISA server can do FBA)
Apparently an option 'Create User' appears under site settings in the
users and permissions column
