HOW TO: form fill + route + auto-populate



All,

I am interested in created a form that an author accesses to create a
"draft" document with about 15 pre-defined fields (text entry,
drop-downs, etc.) that then gets routed to a particular group (upto 5
groups with x recipients per group) based on one of the drop-downs.

If the group approves the route slip, then the draft information is
auto-populated into a formal document that can be a word doc or pdf or
HTML-prinable page (whichever is easiest).

If anyone here has a suggested method, I would like to hear it.
