He wrote earlier that he is getting new versions if he replaces the entire
text of the document after doing Edit in MS Word.
I suspect that knocks out your last two ideas good though they were.

When you view the document's version history and see only one version,
what
does it say at the top of the screen? The two choices are:
Versions are currently disabled for this document library.
Versions are currently enabled for this document library.
If versions are enabled but only one is being saved, take a look in the
"Change Advanced Settings" screen and verify that there is no event
handler
associated with this library. That's a long shot, but worth checking.
Finally, I know that there are third-party tools for managing version
history, that are capable of limiting the number of versions that are
saved.
Is there any possibility that something like this is installed on the
server?

new version. However when I upload with overwrite checked it just

For clarification the actions I am taking:
1. Using a browser I go to the Document Library and then click the
'Upload
Document' link.
2. I then browse for my document and Select it and have Overwrite
existing
files checked. I then click 'Save and Close'
3. The result is if the document already exists in the Document Library
it
is overwritten and when I select Version History for the document it only
has
version 1 which is the new document.

I agree with Joe's suggestion on this issue, you need to enable
versioning
on the document library, and upload a new version with same file
name( With
"Overwrite existing file" enabled). The old version will be
overwritten
and saved as a version.
