I had the same problem while trying to upload a batch of files +50Mb total. I
had also read the KB and made the necessary changes to web.config, to no
avail.
I was able to fix my problem by going to Sharepoint Central Administration >
Configure Virtual Server Settings > select your server here > Virtual Server
General Settings > Maximum upload size. This value was set to 25Mb, which is
why setting the maxRequestLength in web.config alone didn't help.
Have you checked this setting? (Although since you'r trying to upload a
single 200k file, the issue might lie elsewhere...)
