administrator of site cannot connect

An administrator to a site attempted to delete a user, was prompted for a
password failed, and now she cannot get back in the site at all, even to
view??
This happeded once before but I thought it was just one of those things

I rebooted machines and servers...no luck
I deleted her account and recreated (on the site). THe only thing that
fixed it was deleting her account on the domain and readding it?

Any ideas ?
