Depending on the type of item level permissions you wish to implement,
SharePoint's item level security may do what you want. Have you looked into
it?
As far as adding table to the content database -- it is SQL Server, you
could do anything you wish.. Be fore-warned though, Microsoft indicates they
will not support you if you do alert the database in any way.
