The newsgroup for SPS 2003 is microsoft.public.sharepoint.portalserver.
When you post there I suggest you use the words "Custom List" (which I
presume is what you mean by "Form") rather than Form.
The word Form is associated in the minds of most SharePoint 2003 (=both SPS
2003 and WSS) users with InfoPath forms that get added to a SharePoint Forms
Library and as you don't create those "in a SharePoint site", they can't be
what you mean.
You might also care to explain (there!) the Word document piece of what you
wrote below as Word documents tend to be stored in SharePoint Document
Libraries as files - i.e. you have to open them to get at the data.
But please, as above, post such a revised message to the SPS newsgroup.
