We just setup Sharepoint in the DMZ as an Extranet and use it as an Intranet
as well.
Setup a one way trust between your external DMZ domain and your internal
domain.
Add external customers etc to the DMZ domain and give them access
appropriately and add internal users/groups appropriately as well.
A few small issues with this setup but overall works well (if you are ok
with everything technically being an extranet)

I have an Intranet site using Windows Authentication. Everyone can access
the site without any problems when connected to the LAN, but as soon as
are offsite, they encounter problems and timeouts.can
The guidelines I get for exposing SPS2003 site as an Extranet propose I
create another site for the external employees. Is there no other way I
allow both internal and external users to connect to the same site without
duplicating sites?
Also I don't want employees who are logged on the domain to enter their
username and password when they access the site.
