The smtp server is on a completely different server, and I believe that
is working OK as when you create an alert you do get a confirmation
email notifying you of the establishment of the alert. If that works,
I would expect the email of an actual trigger of an alert would work
too.
Also, it's not just the email of a triggered alert that is not working.
I'm not getting anything in MyAlerts either. I can see that the alert
has been established in MyAlerts; but I never get any notification of
changes.

I'm implementing an SPS site for a client and am having trouble with
Alerts.
When I set an alert, I get an email confirming the establishment of the
alert, and that is the last I ever hear. I get no alert, neither in
email nor in MyAlerts, when the target is changed. The Alert topic is
there in MyAlerts, but there are never any actual alerts.
I have set the alerts for immediate. I've confirmed that the
SharePoint Timer system is running and is using an ID that is DBO of
the content database.
What else should I be looking at?
