You can start with an Issues list, and add pretty much any fields that you
want (Modify Settings and Columns).

The Issues List contains a column called "Assigned To." This column gets its
information from the users of the site. When you create a new item and
select a user in the "Assigned To" field, that person gets an email
informating them that an item has been added. Once someone adds an item,
they can choose to be alerted of changes to just that item, not the entire
liste.
