Uploading/Adding documents to library issue

Hi all,

I'm hoping someone maybe able to shed some light on this problem for
me. I have an issue where by when a user adds a document to a doc
library in sps 2003 his/her username/profile is not recorded under the
"created by" or "modified by" columns. This column is left totally
blank.

Has anyone else experienced a similar problem and if so how did they
resolve it?

Any help/tips in the right direction would be greatly recieved as this
one has me well foxed!!!
