Creating a calculated hyperlink

We have a separate issues tracking database, one for each department. As we
collaborate with another department, we're creating a list of issues in
sharepoint which may be tracked in either database. We'd like to have a field
which can be filled in with the issue number, like #27. But then when it is
viewed, I'd like to be able to go to the corresponding web page, like
http://department/issues?id=27.
How can I make it so user just type in 27 when filling in a field but get a
clickable link when viewing it?
