It looks like for each resource it first tries to access it anonymously (the
username pinsley_h is not present). Two entries are logged:
401.2 (Logon failed due to server configuration.) and then a 401.1 (Login
failed).
THEN there appears to be an entry for the same page WITH my username
(pinsley_h). The reply is a
401.5 (Authorization failed by ISAPI/CGI application. )
It appears that whenever it logs an entry for my authenticated username, the
failure is a 401.5.
