    Thank you for detailed description on this issue!

    If you site doesn't grant the permission to one account and disable the
    access request, click the canceal button of logon window will redirect to
    the page
    http:// url>/_layouts/1033/reqacc.aspx?type=web

    You can directly access this page to see whether your IE can render the
    page. If you can't view this page from IE, I'd suggest you can check the
    folder (if you are using the English version, if not, please check the
    coresponding language ID )
    %ProgramFiles%\Common Files\Microsoft Shared\web server
    extensions\60\TEMPLATE\LAYOUTS\1033

    Right-click the 1033 folder to open the folder property dialog. At the
    security tab(ACL: access control list), please grant everyone group to the
    folder to see which account has the permission to access the folder. You
    can try to add everyon perssmission to the folder for troubleshooting.

    If succedd, please check which account your WSS site application pool is
    using and grant the Full control permission to that account in this folder.
    If failed, please check the ACL of the folder Layouts to check the
    permission.

    The 401.1 will be returned from IIS when you input wrong password and try
    three times. This is expected.
