It's a odd coincidence that you should post a problem like this. Eric
Lippert had a recent blog entry on how the host/engine/pdm/mdm/debugger
chain worked.

I posted a comment on his blog on how there is no documentation on how to
debug and fix a broken chain.
I just pinged him by email to see if he can offer any advice or refer it on
to someone on the debugger team...
