User Account Information

I have been asked to retrieve some user account information. Here is a
listing of the information I have been asked to retrieve:

last successful logon
last successful password change
last failed logon attempt
number of failed logons since the last successful logon

I know I will have to query every DC in my domain to get this information.
And I'm pretty sure I know how to get the first three items on the list. It
is the last item I don't know about. Does anyone know how to determine
this?
