I am having similar problems
Have you found any solutions ?
I tried changing the collation no luck.
My fmt file works for with a one field file.
But when I try a two field file with a two field fmt

I get
Server: Msg 4823, Level 16, State 1, Line 1
Could not bulk insert. Invalid column number in format file 'd:\BULKU\G2.fmt'.

My hunch is this has to do with my upgrading to 8.0 and then
uninstalling and going back to 7.0
