Login failed for user 'sa' over and over again

I have a SQL Server 2K with ver. 8.00.818 and already installed the fix for
KB 826161. I checked event viewer and trace the activity using the sql
profiler and i received the following logs every 500 milliseconds;

Event ID: 17055 Category: (4) Description: 18456 : Login failed for user 'sa'
Event ID: 17055 Category: (4) Description: 18454 : Login succeeded for user
'sa'. Connection: Non-Trusted.

How can i prevent this and how to determine the source of this connection
where the hostname from the trace file is blank?
