SQL Report Form Authentication Problem

Dear Sir/Madam,
I have changed the authentication to use Form Authentication based on the
instruction of http://www.microsoft.com/indonesia/msdn/ufairs.asp. After
that, I found that when I tried to login to Report Manager(as a system
admin.) to do admin. task, the login page keep showing and wouldn't redirect
me to the admin. page that I can access before, when I am still using the
Window Authentication. Please kindly help:
01) How to fix the above problem?
02) After register another new user to the Useraccounts db, how to assign
role / grant report to that account?
