Your original question sounded more hypothetical!

In order, I'd start with the SMS 2003 toolkit, run the MP troubleshooting
tool. I'm thinking it may have to do with your IIS installation as there are
accounts needed for IIS to properly function. Given that, remove your MP
role from the site, remove IIS, reboot, then add IIS back in with the proper
components. Then, add the MP role back for your sms site.
