Was this created with "SMS Installer"?
What are the details?
is it a msi or exe?
what are you setting in SMS? run with admin rights or users rights?
Do you have software restriction policies in group policy enabled and
configured with something?
Is this one specific app or any app?
Is this a new package or existing package that you've deployed before?
Is this on one machine or many?
Anyways, if so of those options you haven't tried I would run through those.
Even if you used "SMS Installer" to create this it sounds to me like a SMS
issue, so I would probably post on one of the sms newsgroups. maybe
sms.admin, sms.general, or sms.swdist
