Assuming the maintenance task is enabled, look at the properties of the
client and find out which discovery method is updating the record, from
there you can investigate what is going one.

I also have a customer that has this issue. There are records that appear
to not have been discovered by AD System Group Discovery for more than 8
months, and they are definitely nowhere to be found in AD.
I can't for the life of me figure out why the aren't being deleted...
