SMS2003 - Confusion about MSXML

I'm trying to get my head around where MSXML fits into client installs and patch management on both Advanced and Legacy clients.
Advanced Security mode.
Mostly Win2K/XP pc's, although still significant number of NT4 WS and Servers.
We are mostly interested in "pushing" out software with as little user intervention as possible.
Sources of confusion:
I see this article:

that appears to say that to have a successful patch management SMS system, I need to see that all clients get an MSXML upgrade first.
True?

Wherein it describes how to setup a silent install but I believe that's a bit out of scope. It can't be that convoluted, can it? Is it necessary to buy yet another product (InstallShield) to make a fundamental part of this product work?
And I see the download link:

But I'm not clear as to what needs to be dl'd and how to script it, or feed the right switches to it, to make it run silently.
