In the forms article I referenced, David Bartosik says among other things
that if you want a functional form you have two choices. You can use
Publisher's built-in form capability but "A functional form requires - A)
FrontPage Server Extensions (FPSE) installed on the web server (order from
your web host) and B) your site must be published in the HTTP protocol, and
C) the form properties must be properly configured on the Submit button
control." He goes on to explain that choice further including how to test
for the FPSE, and in the other reference I gave you, explains how to use the
HTTP uploading protocol.
The second choice he gives you ties in with the suggestions Mike has made:
"If your host provides and supports a form program and you have the
technical know how to program it (or your host assists you). In the form
properties dialog for your form simply select the option to use "an ISP
Program" and then input the server path and name of the form program.".
If your webhost does not support FPSE, but does offer a particular "form
program", then they may be referencing the code snippet/script that you need
to insert into your Publisher page, in order for the form to work, or to
actually build the form. In fact they are probably offering one of the
options that Mike outlined, and if they do they probably offer some
instructions you should read. If you choose to use your webhost "form
program" or one of the programs Mike references, and the instructions say
you need to insert some code/script into your Publisher page, you use the
"Insert HTML code fragment" feature in Publisher. Note that you will need to
use Ctrl + V to paste into the Insert HTML code fragment dialog box. Pasting
via right clicking or Edit, will not work.
I would suggest that using FPSE will probably be easier for you, but take
advantage of the links that Mike provided to read about the alternatives. I
would also suggest that you read over the posts on the web design newsgroup
at microsoft.public.publisher.webdesign and the other material David
Bartosik has provided on his site.

Thanks David for getting back to me so quickly. I did read over thethe
references that you gave but it didn't really help. I am not a techie. I
spoke with my host and they told me that I needed to add "scripts" into
publisher page with the form. But I don't have a clue how to do that??message
