I cover most issues on the FAQ page -
http://www.publishermvps.com/Default.aspx?tabid=30

You don't really say what the problem is regarding the uploading, would need
some details. But try the FAQ first.

The file naming convention changed but is irrelevant. You can however use
your own names, in both versions 2000 and 2003. I have this info on my site.
In 2003 go to tools, options, or use the rename button on the web toolbar.

Version 2003 uses only the .htm extension. There is no legitimate reason why
the server should recognize .html and not .htm. Servers are supposed to be
setup to accept index.html, index.htm, default.htm, default.html,
default.asp, default.aspx, as folder defaults, at a minimum (others can be
used). If your host actually only supports one that would scare me into
taking my business elsewhere.
