Sounds to me like it has inserted itself into the Notification, database.
And after each soft reset it is requesting that it run. Problem is either
the card name has changed or the card is not yet ready to be read. You'll
need to do a hard reset or use a program like Pocket Mechanic to delete it
from the Notification query (database).
