I tried another install and re-install of AS4.1 but still the same and
still that blasted 86000108 error.

Microsoft need to patch AS4.1 as it's buggy as hell!

Can you try running Help -> Detect and Repair in Outlook, then
reinstall ActiveSync and try again.
