Two issues with Outlook 2007

1. I want to order the shortcut buttons at the lower left, but they always revert to the default configuration (Mail, Calendar, Contacts, Tasks, Notes). Is there some way to make it stick?

2. I want to create a custom form based on the Notes form. But when I pull it up, I get "The form you selected cannot be displayed". I can open all other forms. Why can't I open this one?
