Did you also change servers as part of the Outlook migration? What version
of Exchange are you using? Can you go ahead and set up an Organizational
Forms library on your Exchange server? I suspect that would solve the
problem. You can leave the permissions set so no one can write items there.

Can they switch to the Personal Forms library after the Choose Form dialog
comes up?

You mean in the Choose Form dialog? No. There are other ways to launch
