SOLUTION - Outlook 2003 - Transformations and Custom Installation Wizard (CIW)

In case it happens to help anyone in the future, I eventually tracked
this down to step 7 of the CIW. In the upper right hand corner is an
option "do not migrate previous installation state." Apparently the
fact I was carrying forward a previous installation state that did not
include Outlook caused Outlook to be disabled by default. Checking
this box and then applying it to the entire branch solved this issue.
Outlook is now installing just fine.
Just an FYI in case it helps someone.
