OK, more info:

*it appears it's really happening when user1 gets an email from user2 (both
internal)
*message is stored in a folder under the inbox
*user1 attempts to forward and/or reply to message from user2
*message does not appear in "sent" items, the "sent" message goes back into
the same sub folder.
*can be duplicated by user3 receiving an email from user2
