I don't see anything in the instructions that are not applicable to Outlook
2002 users, except maybe that it refers to a Compose menu instead of the
FIle | New menu. You can get to the Manage Forms dialog through Tools |
Options | Other | Advanced Options | Custom Forms | Manage Forms

Possibly the easiest way for you to use the form is to follow the
instructions for copying it and copy it to your Inbox. That way it will
appear on the Actions menu of your Inbox.
