Did you publish the form definitions with the form? What version of Outlook
are you using?

After furious head scratching, Radu asked:
| I've developed a form with some checkboxes on it. When I
| send it to my self I get it, but when I start inputing the
| some note in the message box and start checking some of
| the checkboxes, I try to reply and I don't get that page
| anymore.
|
| What I am trying to do is send out this form to some
| users, have them fillout the form and reply to me. I
| should be able to see everything they've done on the form.
|
| How can I do this?
|
| Thank you for your help.
