Custom forms (user-defined field in general) are not supported by the
Outlook import/export wizard - you will need to either write your own code
or use a 3rd party product.
You may find one of our options of interest - both have custom form support

I am working with a new user defined form and am trying to import contacts
into this form. I have already set this form to be used by going to the
properties area for the contact database and also set this database to
update
to this form.
However, when I go to import my contacts, the form fields that show for
the
mapping show the fields for the standard contact form.
How do I get the correct fields to show for the mapping part of the
import?
