That happens when the View is "Details", the icons are sorted by Modified
Date, and the "Show in Groups" choice is checked in the right-click menu
"Arrange Icons By" settings. If you change the arrangement to sort by name,
the groups will be by letter of the alphabet. Or if you sort by size, the
groups will be Tiny, Small, Medium, and Large.

To turn off the headings, right-click a blank area of the list, choose
"Arrange Icons By" and click "Show in Groups" to uncheck it.
