What add-ins do you have installed? Google Desktop Search was known to create a load of extra toolbars but a recent update fixed that issue. Start Outlook one time in Safe Mode and see if you get the same behavior. If not, then restart Outlook normally, disable all add-ins and start adding them back one by one until you find the culprit.

After furious head scratching, project109x asked:
