Outlook form: Action Names in Action tab has been deleted after user preview the form

Hi!,

Since my organization started rolling out Outlook2003, one of my forms
that used to work fine in Outlook XP has been out of order.

The process of the form are as below;
1. user open the main Outlook form (form1)
2. user click on one option to trigger the main form to call for
"form2", which is a hidden form, to open for user to enter information.
3. user will click the submit button in "form2"
4. "form2" reached the Authorizer's mailbox as stated in the code
5. the Authorizer then can choose to "Authorized" or "Not Authorized"
the form by clicking on the upper left "Authorized" or "Not Authorized"
button, these button came from the Action Name setting in the "Actions"
tab.
6. The requester then recieve the reply mail from the Authorizer

The situation right now is that, in step 4, when the form newly arrived
the Authorizer's mailbox, the icon in front of the mail is shown as a
"Post" icon (the icon that looks like a Post-It with the pin and the
middle) instead of the "New Mail" icon and when the Authorizer put the
mouse on that mail record in the mailbox, the preview pane display the
information that has been pass to the Outlook form in clear text.

If the Authorizer double click on that mail to open it right away after
have the mouse over on it, the "Authorize" and "Not Authorize" buttons
will be available for them to use.

If the Authorizer does not open the form that has been preoviewed right
away, instead going to other places in Outlook then come back to the
same mail, the preview pane now will display as a grey background with
the text of "This item contains active content that connot be displayed
in the Reading Pane. Open the item to read its contents." in it as it
suppose to be. However, when the Authorizer open the form at this
situation, the "Authorize" and "Not Authorize" buttons are disapper
from the form and when view this form in the design view, all the
Action Names in the "Actions" tab has been totally deleted.

This hidden form was working fine in the Ourlook XP environment and now
I have to tell the Authorizer to open the form immedietely after they
preview it to prevent the lost of Action Names in the "Action" tab.

Have you ever experience something like this? I really need some help.
