outlook form approval

We want to use outlook forms for users to record gifts received and want to
know if these scenarios are possible using a single form.
The user will complete the form for the gift giving all details including
value of gift.
Scenario1
If gift value is above £25 and below £75, the user completes the form and
sends to pre-entered email box where the data will be extracted to an access
database using an outlook macro.
Scenario2
Gift value is above £75, user enters information but is forced to select
different email address, ie. To address cannot be the pre-entered address.
User enters his managers address and sends. the manager views the data and
ticks a box to approve and sends to email address. Data is extracted to same
access database using an outlook macro
Can this be achieved using a single form? I was thinking that the form could
have 3 tabs - tab1 instructions, tab2 user enters gift details, tab3 manager
approval part.
