I've seen this a lot. Also, a file that has been opened recently (like today
or yesterday) remains in the last week pane in the project gallery.
Permissions are not the issue. However, I never bothered to search out the
real reason or situations when it occurs, but I've reported it several
times. I simply use Excel's list or use file-open, and anxiously wait for a
fix in an update.
