And if you're going to follow John's method, I'd suggest that you do the
Paste Special as PDF rather than as Picture. I think you'll get sharper
results which will be more compatible if the document goes elsewhere... I'm
sure John will correct me if I'm wrong :-)

If Bob's answer does not fill the need, then you need to first create the
page as a "Picture".

To do that:

1) Select all the text and Copy

2) In a blank document, Edit>Paste Special>As Picture

3) Right-click what you have pasted and set the Text Wrapping style to
"Tight"

4) Enter the header area of the source document, select all and Copy

5) Paste that into the blank document too, and set its Text Wrapping style
to "Tight".

6) Do the same with the footer.

7) Now rotate all three pieces and adjust their positions appropriately.

8) Select all three pieces and right-click and Group them (you can't rotate
while they are grouped)

9) Now copy the whole thing and paste it back into the source document (and
delete the original).
