Damn! That's one of those "Can't be fixed" frustrations.
The template has corrupted and for some reason, Word can't find the graphic.
If you care to zip up a copy of that template and email it to
john@xxxxxxxxxxx, I will pass it on to Microsoft where they would like to
see "why" Word can't find the graphic.
In the meantime, sadly you will need to restore your backup.
If this was a Word 2008 template, we could get in and fix it, but not with
Word 2004.
