Formatting index entries

I am polishing a 700+ page geneaology project for a client. In generating the index, I am finding that the names are being alphabetized by first name as marked. Makes sense, but I need it to reverse to:
<LastName>, <FirstName> <Middle>
I looked at switches in the Fields dialogs for Index and XE, but nothing there.
Anyone know an automated solution? I have about 1000 names in one chapter. Could be 10,000 plus in the entire volume.
