Hi Tim!
Copy your Microsoft User Data folder to a different Mac OS X user account and see if the problem persists. This will help determine if the problem is with your Database or with your user account.
Locate the "Microsoft User Data" folder in the Documents folder of your home folder. Copy this folder to /Users/Shared.
Create a new Mac OS X user account under Apple menu --> System Preferences... --> Accounts. Then switch to that account.
Copy the Microsoft User Data folder in /Users/Shared and copy it to the Documents folder of your new Mac OS X account.
Launch Entourage and see if the problem continues.
